While this document will guide efforts for the next 5 years, public health will regularly revisit the plan to ensure it continues to reflect the county's needs and priorities.
Background
The Community Health Assessment is designed to provide a comprehensive analysis of the health status of the community and identifies areas that we can work to improve as a community.
Every five years, all local Colorado public health agencies complete a Community Health Assessment and develop or update their Public Health Improvement Plan. This information is then incorporated into the statewide plan by the Colorado Department of Public Health and Environment.
The assessment reviews a number of different health indicators including air and water quality, acute and chronic diseases and injuries, and social determinants of health including poverty, income and education. It serves as a reference to develop strategies to mobilize the community to address key health issues.
